MISO Youth Forms

Youth Player Registration
Registrations for youth players participating in the MISO Junior League (MJL) must be done through US Club Soccer (USCS) by the Team or Club's registrar, as a login and password will be required. The USCS website is https://usclubsoccer.org/resources/registration/

MISO Juniors Application Form
Applicants should familiarize themselves with the League Rules prior to applying, as there’re a few differences in comparison to other Leagues.
Admission is limited. Returning Clubs/Teams will have priority provided they meet the deadline. All others will be based on a first come first serve basis and/or the teams’ rank.

MISO Adult Forms

Adult Player Registration
HSA Registrations for Adult Players playing in MISO Div I, Div II, Div III, W1, M35+ or Masters Divisions must be done online through the Affinity website. Team Managers usually handle the registrations of their players, but they may opt to have other representatives or players handle the registrations. A login and password may be required. All registrations must first be approved by the team manager of the team the players are registering with. Players may not register without an assigned team. Looking for a Team?
New to the island or don’t have a team yet? Register as a free agent by submitting your information. Any interested team managers will contact you. You can also come to the fields to watch teams play and introduce yourself.

New Team Application
Teams interested in joining should first submit a New Team Application Form to be reviewed or kept on file if no openings are available at the time. First come, first served is often used as one of the criteria when selecting a team to enter the League, but it is not the only criteria used during the selection process. The League also looks at the quality of the organization or team management skills, as well as strength and attitude of the applying team and/or its players, provided they have past history in the League or other Leagues in the State.
